All Students:

Physical and dental health exams from health care providers are required for students entering grades K, 1, 3, 5, 7, 9, and 11 and new entrants. Download forms below:

• The Student Health Inventory Form should be filled out by parents / guardians each year and submitted to the school nurse in the first week of school.  Also available in Spanish.

Food

If your child has food allergies, please fill out the Food Allergy and Anaphylaxis Emergency Care Plan in English or in Spanish. Please return it to the school nurse.

If your child is allergic to any nuts, please fill out the Nut-Free Table Form and return it to the school nurse.

If your student self carries medication for allergies, please fill out the Independent Medication Carry and Use Form.

Bees

If your child has an allergy to bees, please fill out the Bee Sting Medication Form.

To comply with NYS Medication Guidelines, school nurses are not permitted to dispense ANY medication to students without an order from your child's physician. This includes all over-the-counter medications such as Tylenol, Motrin or Advil, allergy medications (Benadryl), cough drops as well as asthma inhalers.

Please have your child's physician complete a medication authorization form for each medication to be given.

A parent must deliver the medication in the original, correctly labeled pharmacy container to the school office or nursing office personnel.

It is essential to call your child in each day he or she is absent or late, even if it is weather related.

Parents are required to provide a written excuse each time a child is absent or late. The note should be sent with the child when he or she returns to school.

• Westlake Middle School - Please email - wmsattendance@mtplcsd.org  or call 769-5500 x 1507 (attendance office) Notification of your child's absence is required; please email or call and include the reason. If the absence results from illness, please mention this in your correspondence. 
• Westlake High School-If your child is going to be absent, please email whsattendance@mtplcsd.org or call 769-8311, prompt 1 or ext.  1507. Please include the absence reason in your email or call. If your child is home due to illness, please indicate that in your communication.

Students under 18 who plan to work need employment certificates ("working papers") before they can start a job. The school must have a current (ie., done within the past 12 months) physical exam on file to issue work permits.

To get started, students should complete the Application for Employment Certificate (AT-17).

During the school year, working papers can be obtained during regular school hours. Contact Mary Jane Au at mau@mtplcsd.org or 914-769-5554.

Students should be prepared to bring proof of age when applying.

Changes to the NYS Education Law require Doctors to include BMI (Body Mass Index) and related Weight Status Category on the mandated School Physical Exams. BMI and Weight Status Category are based on your child’s current height and weight. 

The Weight Status Category provides us with information regarding your child’s BMI-for-age percentile, for example, less than 5th percentile, 25th percentile, 50th percentile, etc. This information allows us to know if your child’s weight is in a healthy range.

NYS Department of Health mandates our district to take part in a BMI/Weight Status Category Survey. We will not be submitting any personal student information on the survey. Names will be withheld. We will only be sending tallied Weight Status Category information which will help health officials develop programs that make it easier for children to be healthier.

You may choose to have your child’s information excluded from this survey report. If you do not wish to have your child’s Weight Status Category included, please complete the form below and return it to the Nurses Office by December 1st. If the form is not returned, your child will be included in the survey.
